Dedham Medical Gets Recognized as Patient-Centered Medical Home

Dedham Medical Associates has announced that both its Dedham, and Norwood practices have received the highest possible national accreditation as Level 3 Patient-Centered Medical Home from The National Committee for Quality Assurance (NCQA).

Dedham Medical Associates has announced that both its Dedham, and Norwood internal medicine practices have received the highest possible national accreditation as Level 3 Patient-Centered Medical Home from The National Committee for Quality Assurance (NCQA).

NCQA’s Patient Centered Medical Home program recognizes physician practices that prioritize the strengthening of the physician-patient relationship, coordinate primary care for patients across multiple settings, and engage in a team approach to improving patient care.

"The patient-centered medical home promises to improve health and health care," said NCQA President Margaret E. O'Kane. "The active, ongoing relationship between a patient and a clinician in medical homes fosters an all-too-rare goal in care: staying healthy and preventing illness in the first place. PPC-PCMH Recognition shows that Dedham Medical Associates has tools, systems and resources to provide its patients with the right care at the right time."

To receive recognition, Dedham Medical Associates demonstrated the ability to meet the program’s key elements embodying characteristics of the medical home. Dedham Medical Associates met key program components in the following areas:

  • Written standards for patient access and continuity of care
  • Use of patient feedback, education, and self-management materials
  • Appropriate use of charting tools to track patients and organize clinical information
  • Responsive care management techniques with an emphasis on preventive care for individual patients and for the entire patient population
  • Use of information technology for prescriptions, tests, referral tracking and coordination with other health care providers
  • Use of evidence-based, clinical standard guidelines to treat chronic conditions
  • Measurement and reporting of clinical performance and patient experience

Dedham Medical Associates joins other Atrius Health affiliates in achieving Level 3 status making Atrius Health one of the largest practice groups in the country to achieve this NCQA medical home designation across 37 of its internal medicine office locations.
