Commonwealth Care Alliance and Atrius Health Announce Collaboration in One Care Program for Mass Health/Medicare Patients

Commonwealth Care Alliance (CCA) today announced Atrius Health is a participating provider in the CCA One Care plan which provides high quality, coordinated care to patients who qualify as “dual eligible” under the state’s One Care: MassHealth plus Medicare program.

The One Care plan, open for enrollment on October 1, 2013, is a new health care option developed by the state for people age 21-64 who are eligible for both MassHealth and Medicare.  With this new option offered by CCA, eligible patients will receive health care coverage under a single plan that will coordinate and integrate all the services covered by MassHealth and Medicare as well as offer additional services.

“Atrius Health and Commonwealth Care Alliance share the same goal – to provide these individuals who have very complex medical needs with better access to integrated care,” said Lois Simon, President of Commonwealth Care Alliance. “A collaboration with Atrius Health was a natural fit for us and allows us to broaden our reach in serving this important population.”

CCA’s One Care plan is one of three One Care plans available across the state. Patients will have a primary care provider who will coordinate their care, and a team to help them with their care decisions.  CCA’s One Care program will offer improved access to a wide range of services, including behavioral health, home and community support, and dental and vision benefits without copays.

“The One Care model is a very complementary clinical model to our patient-centered medical home and Pioneer ACO work for our patients, so we are delighted to be a participating provider in the program,” said Dr. Richard Lopez, Chief Medical Officer of Atrius Health.

Atrius Health’s community-based medical groups include Dedham Medical Associates, Granite Medical Group, Harvard Vanguard Medical Associates, Reliant Medical Group, South Shore Medical Center, and Southboro Medical Group.  CCA’s One Care plan is available to each group’s eligible patients who reside within the CCA service which includes Essex, Franklin, Hampshire, Hampden, Middlesex, Norfolk, Plymouth (partial), Suffolk and Worcester counties.
