The Dedham Board of Health has scheduled a flu clinic for Thursday, October 4, 2012 from 11am-2pm at the American Legion Hall, 155 Eastern Ave, Dedham for anyone 18 years of age and older.
You can get the necessary forms ahead of time on their website: www.dedham-ma.gov, the library, Town Hall, or email mlaroche@dedham-ma.gov.
A copy of your insurance card must accompany the forms.
